How To Submit Success Stories

Objective: Many programs that provide HIV care have found unique and creative solutions to improve the care delivery process through performance improvement activities. The AIDS Institute is interested in creating a forum where examples of Success Stories can be shared and therefore, help to build capacity for quality improvement. Examples of Success Stories provide health care facilities the opportunity to learn and facilitate peer learning among HIV programs.

Definition of a Success Story: Success Stories are storyboard like presentations that represent the work of a quality improvement (QI) team and illustrate the results of successful projects to improve one single aspect of care. The format of Success Stories includes a statement of the quality problem and improvement goal, performance measurement data over time, a description of the changes tested, and lessons learned in the process.

Success Stories should reflect the results of a variety of different projects including:

  • improving clinical aspects of care currently reviewed through IPRO or HIVQUAL, such as GYN care, rate of PPD readings, PCP prophylaxis;
  • facilitating coordination of care within the clinic or with subspecialty services;
  • increasing the efficiency of patient flow in the clinic;
  • improving non clinical aspects of care;
  • engaging and empowering patients to be more involved in monitoring their own care, and
  • improving documentation standards through innovative ideas and the use of new technologies.

Format: Success Stories should be submitted using the attached standardized form. This form is similar to the storyboard format that is used for many conference presentations. It includes general information about the HIV program, background facts regarding the QI team, specific information about the quality improvement project such as problems identified, changes tested, performance measurement data over time, and lessons learned. Please submit an appropriate photograph of your facility or staff if you would like it to appear with your submission. (Please note that if a client appears in the photograph, a confidentiality release should accompany the photograph).
