Case Management
HIV Case Management
Case Management programs provide client-centered services that link individuals with primary medical care and health-related support services to ensure timely, coordinated access to the full continuum of care. Case management professionals assist clients access various services including medical care, housing, food, substance abuse treatment, home care and mental health services.
Background
In 2002, the HIV Quality Management Program convened Ryan White Title I-funded case management providers to develop HIV case management performance indicators. Data were first collected for calendar year 2002. In 2003, case management providers began to participate in QI Learning Network meetings and receive QI technical assistance to develop projects to address quality improvement in their service programs.
