Tools for Integrating Prevention into the Care of HIV-Positive Individuals
The challenge of effectively integrating prevention into the HIV health care environment has become an important priority for both clinical care and public health. People living with HIV who are faring well on long-term HAART face the challenges of preventing further harm from risk behaviors and preventing comorbidities that may occur independently of HIV. Together, these phenomena demonstrate the essential role of prevention education within the HIV healthcare system.
Based on recommendations from the HIV Quality of Care Advisory Committee, the HIV Quality of Care Program launched a campaign in January 2005 to promote prevention activities. Nearly 200 facilities were expected to conduct a QI Project during the calendar year 2005 and to develop a storyboard for submission to the AIDS Institute at the end of the year.
The New York State HIV Quality of Care Program, as part of its campaign to improve integration of prevention into HIV care is pleased to share examples of surveys, forms, and tools used by HIV providers and presented as part of their storyboard submissions. While these tools are not officially endorsed by the Health Department, they have been successfully used in many sites and may help providers to implement prevention QI programs of their own.
